1999 Nissan Quest.....engine dies, sputters, and knocks!?

My Quest minivan is sputtering, choking out, and knocking. When I get it started, it won't go over 5 miles an hour, and then it sputters and chokes out. I had it in the shop, and they replaced something in the cam, but this didn't do the trick. Any ideas what it could be?
